your full name and address as it appears on the check.
Include your phone number and email address. |

Shipping Address - If you wish to receive your
order at a different location than that of the billing
address, please include it with your order. |

Driver's
License Number - Requested for your security. |

Void
out the Check in your Checkbook - For your records,
we
suggest you fill out the check as you would for any
other purchase. |
